4C Health Benefits

  • 4C Health offers two plan options to employees and their families to ensure that participants can choose the best fit for them.

    Medical Expense Plan A Deductible: Single $2,500, Family $5,000

    Medical Expense Plan B Deductible: Single $6,000, Family $12,000

  • 4C Health offers dental coverage for routine and preventative services as well as coverage for basic and major procedures through Unified Groups. Schedule of benefits here

  • Vision coverage is available for 4C Health associates and their family provided by VSP. Coverage includes annual eye exams and an allowance to spend on glasses and/or contacts. Plan details here.

  • Qualified 4C Health associates are provided with Basic Term Life and Accidental Death and Dismemberment (AD&D) insurance at no cost to the employee through Lincoln Financial Group. You may purchase additional coverage for you and your dependents at competitive rates.

  • Short Term Disability

    Short term disability provides qualified associates with short term income protection if he or she is unable to work due to covered accident or illness. See schedule of benefits here

    Long Term Disability

    Long term disability is intended to protect an associate's income for a period of up to 24 months after he or she has depleted short term disability benefits and any accrued paid sick time. Additional benefits details here.

  • 4C Health associates are eligible for a 403b Retirement Plan.

  • 4C Health offers flexible spending accounts that allow employees to save money pre-tax for medical expenses and/or childcare expenses. At the end of the year, if there are funds remaining in your account, you can roll-over up to $610 to the next plan year for calendar year 2023.

  • Health Savings Accounts are tax-exempt accounts that are employee controlled and employee funded. Funds from employee contributions can be used to pay for qualified out-of-pocket expenses not covered by the agency health plan. Employer contributions are made on a monthly basis by 4C Health for Wellness participants, not to exceed $250 for single plan participants, $500 for Employee + 1 plan participants, and $750 for Family plan participants.   • 4C Health offers a generous Paid Time Off benefit! AND get your Birthday off after 1 year of employment

    4C Health recognizes eight days during the year as paid holidays for eligible associates.

  • As a provider of mental health and addiction services, we understand the importance of access to counseling services for you and your family. We partner with Lincoln Financial Group to offer EmployeeConnect services 24/7 provided at no cost to associates. Benefit details here.

  • 4C Health encourages continuing formal education and partners with associates to provide partial financial support for out-of-pocket tuition expenses. Reimbursements are paid based on the grades earned and associates can earn reimbursements up to 90% of the out-of-pocket tuition costs, not to exceed an annual maximum of $2,500 per calendar year and a lifetime maximum of $10,000.

    Achieve Your Degree (AYD) is a tuition deferment program for 4C Health associates who wish to pursue an approved credential with Ivy Tech. Participants work toward earning stackable credentials: beginning with a Certificate that is a component of the Human Services Technical Certificate and Associate Degree, or other approved program.

  • 4C Health’s policy is to encourage and reward successful degree recipients by means of student loan repayment assistance through a loan repayment assistance program. Payments are made directly to the loan holder in the amounts of $150 per month for a maximum of $1,800 per year, not to exceed the lifetime maximum of $10,000.

  • 4C Health offers a monetary referral award to an associate who refers a friend, family member, or professional acquaintance who is subsequently hired by 4C Health. See program details here.

  • 4C Health offers the following Aflac Group Insurance plans to associates and their dependents; Accident, Hospital Indemnity and Critical Illness. Plan details here.

  • 4C Health associates interested in learning more about tobacco cessation or are ready to embark on the tobacco quit journey may find these below resources helpful. This is not an exhaustive list of available resources.
