4C announces opening of new Logansport Clinic

NOTICE OF UPCOMING CHANGES IN LOGANSPORT CLINIC:

Greetings to all our clients and partners,

It is with great excitement that we announce the opening of our new Logansport Clinic location at

800 Fulton Street, Logansport, IN. As of June 5ᵗʰ, 2023, ALL Outpatient Services will be located at

this address. 4C Health has operated out of the previous location for many years and we are

extremely excited for everyone to come and experience our brand-new clinic location to receive

their in-person services, such as therapy, medication clinic services, psychological testing, and

groups!

Over the next couple of weeks, 4C Health will continue to see individuals at the original location.

Then between the dates of May 22ⁿᵈ-June 2ⁿᵈ outpatient services in Logansport will move to virtual

with a few exceptions listed below. This will allow for a smooth transition into the new Logansport

Clinic.

Current through May 19ᵗʰ: Services will remain at 1015 N. Michigan Ave, Logansport.

May 22ⁿᵈ-June 2ⁿᵈ: Appointments will be limited for in-person services occurring at 1015 Michigan

Ave, Logansport, for these last 2 weeks. You will receive direct communication as to whether your

appointment has been moved to virtual or remains in person.

June 5th and forward: All outpatient appointments in-person will be at the new clinic location of

800 Fulton Street, Logansport. Park and Enter on the Westside of the building.

Special Note: Acute Care Unit and Crisis Stabilization Unit remain open at the 1015 Michigan Ave,

Logansport. Use the Main Entrance and push the doorbell, located on the left side in the entry way.

This will connect you to the Acute Care staff immediately.
