4C Health Updates Role Title to Better Reflect Recovery-Oriented Care
North Central IN— 4C Health is pleased to share an important update reflecting the continued evolution of our services and the work of our dedicated team. Effective immediately, the role formerly known as Skills Trainer will transition to Recovery Facilitator.
This change represents a more accurate and holistic description of the support provided to the individuals and families we serve. While the term “recovery” is often associated solely with substance use, at 4C Health it reflects a broader journey—one that includes healing, personal growth, resilience, and overall well-being. The updated title better aligns with our recovery-oriented approach and the way our team walks alongside individuals as they progress toward their goals.
Why “Recovery”?
Recovery represents hope and the desired outcome for every individual we serve. It reinforces our belief that people have the capacity to heal, grow, and achieve meaningful, sustainable change—regardless of where they are in their journey.
Why “Facilitator”?
The role has evolved beyond a sole focus on skill development. Recovery Facilitators support individuals through both essential life-skill building and the facilitation of case management needs. This includes helping individuals navigate barriers, coordinate resources, and access the supports necessary to thrive within their communities.
This updated title underscores 4C Health’s commitment to person-centered, recovery-focused care and reflects the impactful work our team provides every day.
Dr. Carrie Cadwell, President and CEO of 4C Health, states:
“To date, Community Mental Health Centers defined the role of Case Manager or Life Skills Trainer based primarily on the predominant service type being provided. Under the Certified Community Behavioral Health Clinic model, we saw an opportunity to step away from that approach and define the role based on where it fits within the care system and the care team. Hence, Recovery Facilitator was born. This role is equal parts interventionist, case manager, and care coordinator. Much like a conductor—teaching needed skills, pulling all the pieces and people together, and facilitating shifting emphases to achieve recovery. Like a conductor, the Recovery Facilitator does not exist alone; they exist within the team and within the ecosystem of the patient’s needs to support recovery from mental health and substance use concerns. We strongly believe this model provides clearer role definition for staff and leads to better outcomes for our patients.”
