Patient Discharge

Hello,

You have been directed to this letter because you have either requested or are being recommended to be discharged from services from 4C Health.

If you wish to continue services with 4C Health, please call 1-800-552-3106 to either confirm or schedule your next appointment. Otherwise, it will be assumed that you agree with being discharged from services. 

If services are ended, 4C Health will remain available to provide services on an emergency basis. Please call 1-800-552-3106 or 988 anytime, or walk-in emergency services are available 24 hours a day at 1000 N. Broadway in Peru, as well as during business hours in all clinic locations. 

A copy of your Discharge Summary can be provided. Please call 574-732-2451 any time to request this documentation.

Should you decide later to return for additional services, please feel welcome at 4C Health after completing the intake process again. If you schedule an appointment after receiving this outreach letter and no show that appointment, we will proceed with your discharge.
