4C Health Strengthens Partnerships with Managed Care Entities Through Expanded Transparency and Data Sharing

Enhanced collaboration reinforces commitment to improving outcomes, strengthening care coordination, and advancing whole-person care across Indiana.

North Central, IN – 4C Health is proud to announce an important advancement in its ongoing collaboration with Indiana's Medicaid Managed Care Entities (MCEs), including Anthem, CareSource, Managed Health Services, and United Healthcare Community Plan. Building upon years of collaboration, 4C Health has expanded its commitment to transparency by providing comprehensive reporting on care outcomes, service utilization, and performance data designed to improve the quality, coordination, and effectiveness of behavioral health services across the communities it serves.

As a Certified Community Behavioral Health Clinic (CCBHC), 4C Health has long worked alongside managed care organizations to ensure individuals and families have access to timely, high-quality, and integrated behavioral healthcare services. This enhanced level of collaboration further strengthens those partnerships while creating new opportunities to identify needs earlier, coordinate care more effectively, and improve health outcomes for individuals throughout Indiana.

Through this initiative, 4C Health is sharing outcome measures and key performance indicators that provide greater visibility into access to care, engagement, quality metrics, and other factors that influence overall health and wellness. By strengthening the exchange of information, 4C Health and its partners can make more informed decisions and better address the unique needs of the communities they serve.

“Our journey as a CCBHC over the last 19 months has really led us to understand that optimizing care outcomes for Hoosier Medicaid Members under this new model requires a distinct level of commitment to data-sharing and coordination with our Medicaid Managed Care partners. Each year we want to be better and do better on behalf of and with our consumers and communities. To do that, it means taking CCBHC to the next level with health plan partners. The data you see shared today is 4C Health data. It allows our health plan partners to understand how we are performing on critical CCBHC measures for their members. From here, our goal is to get to patient outcome measures.”
